  1. General. Academic Oncology Resources, LLC (the "Company") operates a website, FindCancerExperts.com (the "Website"), that provides a pathology referral service for second opinions on cancer diagnoses. In order to help the Company select qualified pathologists for the subscriber, the Company asks the subscriber a limited series of questions about his/her cancer diagnosis. The questions are narrowly tailored to obtain only the information necessary to assess the subscriber's needs. Failure to provide this information will limit the functionality of the Website referral service.

  2. Company Privacy and Security Mission Statement

    The Company will use its best efforts to:

    1. Hold and use the personal information for the sole purpose(s) specified in Paragraph 4 below and for no other purpose(s).
    2. Hold only personal information that is adequate, relevant and not excessive in relation to the purpose(s) specified in Paragraph 4 below.
    3. Ensure that personal information is not kept longer than is necessary.
    4. Provide, upon request of a subscriber, access to that subscriber's personal information.
    5. Adopt appropriate security measures against unauthorized access to, or alteration, disclosure or destruction of, personal information and against accidental loss or destruction of personal information.


  3. Privacy and Security Policy. The Company has adopted this Privacy and Security Policy to protect the confidentiality of subscriber personal information and to establish the guidelines by which such personal information is used, processed, transmitted, stored and deleted.

  4. Use of Personal Information.
    1. Medical Information. The Company will use the personal medical information disclosed to the Company to solely (i) determine which referral physicians in the Company's network are most qualified to provide a second opinion on the subscribers's cancer diagnosis (the Referral Service). The Company will obtain the consent of each subscriber to the Website authorizing the Company's use of the personal medical information for the purposes described above. The Company will not use a subscriber's personal medical information for any other purpose without the subscriber's written consent.


  5. Information Processing and Storage.
    1. Medical Information. The Company will obtain the personal medical information of the subscriber through its Website by asking a series of limited questions. The Company will only disclose identifiable personal medical information to its employees and agents, and then only to the extent necessary, for the purposes described in paragraph 4 above. The Company will not disclose identifiable personal medical information to any other person or entity without the subscriber's express written consent.


  6. Security. The Company will use encryption software to ensure that any personal data the Company receives from the subscriber is stored securely. The Company intends to use SSL (secure socket layer) encryption, but may change its encryption selection if it believes that an alternative security measure has equivalent or better capabilities. The Company has and will enter into confidentiality and privacy agreements with all of its employees and agents to protect the privacy of the personal information disclosed to the Company. Employees and agents who have access to the subscriber personal information are assigned various levels of password protection. Despite these security measures, the Company does not guarantee or warrant that the personal data will remain strictly confidential and will not be disclosed to any other person due to human and/or machine errors, actions or omissions.

  7. Advertising; Third Parties; Links. The Company does not permit any form of advertising on its Website. The Company will not disclose, transmit, or otherwise reveal any subscriber personal information to outside third parties (other than to the Company's agents and employees for the limited purpose(s) described in Paragraph 4 above) without the subscriber's express written consent. The Company's Website does provide numerous links, which are outside of the Company's control. This Privacy and Security Policy does not apply to any linked website and the subscriber is encouraged to read the privacy statements of any such linked websites, since they may differ from this Privacy and Security Policy.

  8. Disposition of Personal Information.
    1. Medical Information. The Company will store the identifiable personal medical information of any subscriber for as long as the subscriber is actively communicating with the Company through its Website, but in no event longer than twelve (12) months. After twelve (12) months, the Company must obtain the consent of the subscriber to continue to retain any identifiable personal medical information. The subscriber may request at any time that the Company dispose of his/her personal medical information, and immediately when the Company receives this request, it will dispose of the identifiable personal medical information.


  9. Inquiries for the Subscriber. The subscriber may inquire as to the Company's use, transmission, storage or disposal of his/her personal medical information. The subscriber may inquire as to the Company's specific security measures adopted to protect each subscriber's personal medical information. The Company will respond to each such inquiry promptly and completely.

  10. Modifications to this Privacy and Security Policy. The Company may from time to time revise or update this Privacy and Security Policy. Any modifications to this Privacy and Security Policy will be posted on the Website and upon posting, the modifications will be effective immediately. All subscribers are encouraged to periodically click on the Privacy and Security Policy hyperlink to review the then current policy.
